Skilled Nursing
Our skilled practical and registered nurses will deliver a number of quality healthcare services which include:
- Wound care
- Post-surgical care
- Disease management
- Diabetes management etc.
We make sure that proper procedure is done by professionals in providing catheter care, IV and respiratory treatments, infusion therapy and others.
Speech Therapy
Speech Language Pathologists focus mostly on evaluation and treatment of swallowing, communication, and congnitive deficits. These can be from a result of a variety of disorders including strokes, brain injuries, and nuerological diseases. The therapy plan is tailored to each patients individual needs and goals; goals are made based on both the result from evaluations, and on discussions with patient-centered care. The advatage of speech therapy is it can be performed at an individuals home; this allows for treatment of cognition, communication, or swallowing goals in a comfortable and functional setting. Therapy at home can be especially important for people suffering from dementia or cognitive impairment, as a familiar environment can be benificial for participation.
Medical Social Worker
Medical social workers create long-range plans, such as advance care planning and educate both patients and their families about community resources and support. Medical social services include:
- Assessing social and emotional factors
- Advanced care planning
- Community resource planning
- Advanced directives
- Crisis intervention
- Counseling
- Teaching regarding coping with loss and change
Certified Home Health Aide (CHHA)
A trained and certified healthcare worker who provides assistance to a patient in the home with personal care (as hygiene and exercise) and light household duties (as meal preparation) and who monitors the patient’s condition.
Registered Dietitian
Assess clients’ nutritional and health needs
- Counsel clients on nutrition issues and healthy eating habits
- Develop meal and nutrition plans, taking clients’ preferences and budgets into account
- Evaluate and monitor the effects of nutrition plans and practices and make changes as needed
- Promote healthy lifestyles by speaking to groups about diet, nutrition, and the relationship between good eating habits and preventing or managing specific diseases
- Create educational materials about healthy food choices and lifestyle
- Keep up with or contribute to the latest food and nutritional science research
- Document clients’ progress
